There may be other ways to get the audio to work properly, but the only combination of mods that worked were these two (at least for me). I found that any other combination of mods muted the end of the hold.

Probably don't use the REAL PDA-FT Skin as it isn't suitable for non-arcade inputs such as the holds.

Also, I tried to make this chart keyboard friendly.
